Published by CMS on November 14, 2025. Standard Part B premium: $202.90 a month (up 9.7% from $185.00 in 2025). The first threshold rose from $106,000 to $109,000 (individual) and from $212,000 to $218,000 (joint). Amounts are per person, per month.

How to read the boundaries

Each threshold belongs to the lower tier: a single filer with exactly $137,000 of MAGI is in tier 1, not tier 2. The exception is the top: “less than $500,000” for tier 4, “greater than or equal to $500,000” for tier 5. SSA’s Form SSA-44 writes the same limits in cents: “$109,000.01 – $137,000.00”, “More than $499,999.99”.

MAGI is your adjusted gross income (Form 1040, line 11) plus tax-exempt interest (line 2a). It is compared with the threshold without any rounding.

Frequently asked questions

Is the 2026 IRMAA based on 2024 or 2025 income?
2024. The table applies to “MAGI in 2024 (or 2023 if 2024 is not available)”, in SSA’s own words. 2025 income will set 2027 premiums.
Why is the married-filing-separately table so harsh?
The law subtracts the individual threshold ($109,000) from every other dollar amount. The tiers between $109,000 and $500,000 collapse, so income just above $109,000 lands directly in tier 4 ($649.20 a month for Part B). It applies only if you lived with your spouse at some point during the tax year. If you lived apart all year, SSA uses the individual table, but you must tell SSA (POMS HI 01120.060).
How are the IRMAA amounts calculated?
Normally a beneficiary pays 25% of the cost of Part B and the government pays the rest. With IRMAA, you pay 35%, 50%, 65%, 80% or 85% depending on the tier (Social Security Act §1839(i)(3)). That is why the tier-2 Part B adjustment ($202.90) equals the standard premium: at 50%, you pay 25 points more than the standard 25%, that is, a second standard premium. The five adjustments are 10%, 25%, 40%, 55% and 60% of the full cost of Part B.
Do the thresholds rise every year?
The four lower thresholds are indexed to the CPI-U (all urban consumers) and rounded to the nearest $1,000. The top thresholds ($500,000 and $750,000) have been frozen since 2019 and start being indexed in 2028. See the 2027 projection.
Does the late-enrollment penalty come on top?
Yes. SSA’s table says the Part B premium is “plus all applicable surcharges”, and the Part D amount is “plus late enrollment or reenrollment fees”. People with a Medicare Advantage plan that reduces the Part B premium get that reduction subtracted.
